What is the Behavioral Incident Report?

A Behavioral Incident Report documents incidents that involve target behaviors specified in an individual's Behavior Support Plan (BSP), antecedents, the behavior itself, consequences, and the response strategies used. The report is critical for two reasons: it establishes the data that drives BSP refinement (and FBA reauthorization), and it documents that staff followed the BSP as written. Most state DD-waiver programs require behavioral incident documentation linked to the BSP, with monthly or quarterly summary reporting to the behavior consultant.
