What is the Admission Form & Orientation Packet?

The Admission Form documents an individual's entry into a residential, day, in-home, or sponsored-residential service. It captures demographics, emergency contacts, medical providers, allergies, current medications, primary diagnoses, guardianship status, and consent for services. Most state DD-waiver programs also require an admission orientation packet, the suite of acknowledgements, consents, and rights statements that must be signed at admission. Together, the admission form and orientation packet form the foundation of every individual's record.
