Smart Rephrase rewrites any note into DBHDS-aligned professional clinical tone. It corrects grammar, removes diagnostic guesses, tightens vague descriptions, and translates from Spanish if needed. It's everywhere DSPs write - daily notes, ISP outcomes, behavior data, custom forms.

When to Use It

Always

6 Before / After Examples

1. Diagnostic guess → observation

Before: "John was being manipulative again at lunch."
After: "At 12:15 p.m., John repeatedly asked staff to bring his preferred soda after being told water was the option for lunch. Behavior persisted for ~5 minutes; redirected with a choice of two activities."

2. Vague time → clock time

Before: "Earlier in the shift Sara took her medication."
After: "At 8:00 a.m., Sara was administered her morning medications per the MAR."

3. Group note → individual note

Before: "All residents had a quiet afternoon."
After: "Marcus engaged in his preferred art activity from 2:00–3:30 p.m. without prompting. He completed two pages of his coloring journal and asked for a snack at 3:35 p.m."

4. Spanish → English

Before: "El individuo se quejó de dolor en el pecho durante la cena. Le informé al supervisor."
After: "At dinner, the individual reported chest pain. I notified the on-call supervisor immediately and documented per protocol."

5. Future tense → present documentation

Before: "Will work on community skills tomorrow."
After: "This shift focused on in-home self-care routines (Outcome 3). Community-skills practice scheduled for the next shift per the support plan."

6. Tightened ISP outcome

Before: "John will work on being more independent."
After: "By 2026-12-01, John will independently complete morning self-care (8 of 10 days) without verbal prompts."
