5 Common First Forms to Build
1. Admission orientation packet
Combine rights notification, photo consent, communication preference, and emergency-contact verification into a single multi-section form. Magic-link signing for the guardian; staff signature for the admitting program manager.
2. Vehicle inspection log
Pre-trip checklist (oil, tires, brakes, lights, seatbelts, first-aid kit), photo upload for any issues, driver signature. Submit per shift or per week per your policy.
3. Staff training sign-off
Course title, completion date, score (if quizzed), supervisor verification, staff signature. Feeds into the Staff → Certifications dashboard.
4. Vendor / contractor agreement
For one-time service providers (snow removal, cleaning, pest control). Capture insurance info, scope, dates, signature.
5. Internal incident addendum

Best-Practice Checklist Before Publishing
- Form has a clear, descriptive title (not "New Form 1")
- Required fields are marked required
- Long-form fields have helpful placeholder text ("Describe what you observed in 2-3 sentences")
- Sections grouped logically (don't jump from medication to fire safety to demographics)
- Signature is the last field
- Tested by submitting one yourself - every field works as expected
- If guardian signature required: confirmed magic-link works on a test email
